Guidance on Hospitalization

Admission Procedure
  • On the day of admission, please visit the “Admissions Desk (No. 33)” on the first floor.
  • Please make sure to bring the following items at your admission.
    • Health insurance card and various certificates for medical care
    • Name stamp (inkan)
    • “Forms and documents required for the admission” that we provide to you in advance
  • Once we process the paperwork, you can go to the ward.

  • Name stamp (inkan)
  • Health insurance card (If you do not show it to us, you are liable for all medical expenses)
  • Forms and documents required for the admission (e.g., Hospital Admission Application Form)
  • Current medications and their information documents
  • Writing tools and materials

Items that you need to bring if you have or would like to use

  • Various beneficiary certificates (e.g., Eligibility Certificate for Ceiling-Amount Application, Beneficiary Certificate for the Elderly, or Treatment Certificate of Specific Diseases)
  • Discharge certificate

Items that you need for hospitalization

  • Grooming kit (e.g., basin, toothbrush, comb, and soap)
  • Chopsticks, spoon, mug, and handy pot
  • Indoor shoes (non-slip and quiet)
  • Pajamas
  • Underwear, hand towels, and bath towels

You may need to prepare other items. Please follow your nurse’s instructions.
Some of the items listed above are available in the convenience store LAWSON on the first floor.

Special Notice

Smoking is prohibited on the premises. Do not smoke anywhere, including in the parking area.
We often have emergency admissions; therefore, we may ask you to move to another room based on bed occupancy.
When you need to go out or stay out overnight while hospitalized, you must obtain the permission of your attending physician.
Do not bring in valuables or excessive cash. If you need to do so, be careful not to lose them. A “safety box” is available in your room.
Use of mobile phones is permitted only in a limited area.

Rules about the use of mobile phones and devices in each area

•If you are near the area for limited mobile use, you may be asked not to use mobile devices.

(2) Patient rooms Private rooms •Regardless of the presence of medical equipment, you can use mobile devices.
Shared rooms × •You cannot talk on a mobile phone, whereas you can use e-mails and web browsers with good mobile etiquette.
(3) Consulting rooms ×
(No need to power off)
•You do not need to power off your mobile devices.
•Refrain from using mobile devices wherever possible to avoid disturbing a consultation or bothering other patients.
(4) Operating Rooms、ICU、Laboratory、Treatment Rooms × × • You must power off your mobile devices.

 

Information about Private Rooms

If you would like to use a private room, please inform us when you apply for admission.

Four-bedded room (no extra charge) / Two-bedded room (3,300 yen)

Private room (Large:8,800yen / Small: 6,600 yen)

Please note that you are liable for all extra bed charges based on the number of days of use. (Example) If your hospital stay is two days and one night, your extra bed charge is calculated for two-day use.


We prioritize your preference as much as possible; however, the type of room you select may not be available for certain reasons or you may be moved to another room or ward depending on the type or state of your illness.

If you take any medicines, please bring all of them and their information documents.
If you have a prescription record pocketbook (Okusuri Techo), please bring it with you.

Depending on your clinical condition, we may ask you to prepare items other than the above.

 

Life in Hospital

Meals

We serve three meals (i.e., breakfast, lunch, and dinner) at the following times.

  • Breakfast: 7:30
  • Lunch: 12:00
  • Dinner: 18:00

Green tea is available.
Please feel free to use a water heater in the lounge on each floor.

Laundry

Please use a laundromat (coin-operated washer and dryer) in the laundry room in each ward. Please buy laundry detergent by yourself.

Bathing

Bathing may not be allowed depending on your treatment, surgery, or condition. Please ask your physician or nurse.

Lights-on and Lights-out Times

  • Lights-on time: 6:00
  • Lights-out time: 21:00

Mailing

Stamps and postcards are available in the convenience store LAWSON on the first floor.

 

Request for Various Certificates or Medical Certificates

If you need medical certificates or other various certificates, please talk to us at the Document Request Desk of the General Reception Desk on the first floor.
Please note that document preparation takes approx. 2 weeks after you request them.
